This information is intended to assist the school in case of any medical situation involving your child. Please complete the form below if there is a change to your child’s medical condition at any time.

Completing and signing a Medication Agreement for Education and Care Form by your GP is a legal requirement, and it needs to be updated annually. Only medication that is approved, in date, supplied by you, and listed on the form can be dispensed by staff at our school. Any outdated forms or medication cannot be dispensed by staff at our school.

It is also important to note that the School Policy states, that without completed forms or medication your child will not be permitted on school excursions or camps.

A copy will be held by the school within your child’s student record folder, our First Aid Room and will also be available to staff via our student database.
